Putting PREREQ=dmsetup in /usr/share/initramfs-tools/hooks/mdadm and doing an
update-initramfs didn't help.
This is initramfs-tools 0.120 with jessie and wasn't a problem with
initramfs-tools 0.109.1 under wheezy as fa

I installed Debian Etch via expert mode from the RC1, chose not to
install a kernel when it asked, then installed

If $foo_v4l is going to be patched, is there any reason not to patch
$bar_v4l at the same time? It seems more than a little odd to drop one
module for odd-numbered kernels, yet to happily patch a similar module
from the same developer.
