I have same problems but on dum0, server reboots forever, by adding
no reboot the screen goes black just after the hypervisor loads (instead of
reboot)

4.9.0-6 worked fine

and pvh-enabled xen domain are refusing to start.
Nothing useful in the log, it just says the domain crashes

oops also on my box, I doubt it works with debian config, I suppose
this driver is not much used/tested, it works fine for my with an usb
serial device

I've encountered same problem with with initramfs-tools on a pcengines
alix, kernel is linux-image-3.14-1-486
my workaround was adding:
/etc/initramfs-tools/scripts/local-top/sd_mod_bug
that calls modprobe sd_mod
