Looks like I may have spoken a bit too soon. The problem is partially fixed,
but I'm still having a few problems. After the system was up I ran cardctl
eject 0 to stop and let me remove my Ethernet card and I got the follow
error:
eth0: no IPv6 routers present
irq 10: nobody cared!

This issue is reported to the upstream developers, and still pending.

I have just upgraded one of our production boxes from
kernel-image-2.4.26-2-686 to kernel-image-2.6.8-10-amd64-k8 - partly
because we just upgraded to an AMD64 processor.
However, after booting on the new kernel, iptables will not run:

After poking at this, I believe the attached patch represents the best
available heuristic for choosing lvm10 vs. lvm2 for the initrd, given that
(AIUI) there is no apparent way to query the type of an individual volume
from userspace.
