I don't remember exactly, but I had quite the same issues with fglrx
on my 4870x2 at some moment. I've just switched to opensource driver
and enjoyed faster compositing that I had with fglrx:)

If I'm not alone, then it's definitely the bug that must be solved.

I'm using 2.3 now and I don't have any of these issues.
I'm currently using all KDE/Qt related stuff from sid/experimental.
Third one may be related to your phonon backed. Try to choose another.

If removing amarok config doesn't help please try to run "amarok --debug"
from the console and look at the messages.
Looks like you have some strange file, which is scanned during that "12%"
moment.
