for apache, which would allow you to configure most
(or even all) of apache, would be nice too (although I am not quite sure
how difficult that would end up being).

I am using pam_ldap without any trouble on a test system running
testing. It has been some while since I set the thing up, but it was
pretty straightforward, once all the ldap entries were created.
Have you checked what your auth.log says?

You can deactivate it by:
echo "0" > /proc/sys/net/ipv4/tcp_ecn
I hope it works, it helped me last time my packages where rejected.
