I finally tracked this down to a bug in the Xorg server. Sometime around
2009 the XTestFakeMotionEvent() interface was broken and it ignores the
'screen' input parameter.
A workaround is to use the this cmdline option:
x11vnc -xwarppointer
to have x11vnc use XWarpPointer() instead
