The template file then had no Indices-* lines in it, but was otherwise

I changed the title since I have been able to duplicate this with two very
basic Latin-1 encoded RTF files, that were simply .doc files resaved as .rtf
files. wvHtml handled the .doc version correctly, but gave nothing on the .rtf
versions. This should probably be forwarded upstream.
