First, this problem is present in Debian Linux 7.0 (wheezy).
Second, the sequence numbers does NOT matter. The only thing that made
a difference was to execute "update-rc.d" twice ( remove +
defaults).

We have a commercial product which is installing the startup links on
2,3,4 and 5 runlevels with S98. Because of this the service is not
started automatically at boot on Debian 6.0 systems.
