happens when the BIOS doesn't have "USB legacy" enabled to do
USB keyboard support from the boot environment; another possibility is
that the USB keyboard driver and the USB storage driver in the BIOS
conflict with each other.

c126c28095caf13d164b21f4c7cb25a44af859b0, which is included in the
Syslinux 4.00 final release.
