in my case the wrongly selected DVD drive was provided via USB. Was that true
in your case as well?
If so that could be the source of the issue and also why it wasn't possible to
reproduce via qemu.

$syslog +busybox-syslogd
This is required by insserv for the proper order of init scripts.
