Following a hint from Matt Neuburg, I managed to solve this problem by
deleting ~/Library/Caches/com.apple.helpui.
Incidentally, deleting this file also seems to help when I have
another problem. If I delete and regenerate the help book HTML, the
old help book is still displayed by the pro
