That script is listed only under
4.5.6. Prepare and Start Additional Management Servers
Since I was not planning on setting up additional management servers at
this time, I skipped that step.

I do see the 'system' and 'admin' usernames in both the 'account' and
'user' tables, so it's possible the issue you refer to is not the same
problem I'm experiencing. Looks really close, though. :)

Not sure. It's always easier when you're familiar with the process, but I
recently went through a setup via those docs and it came out fine. Perhaps
a step somewhere failed and you didn't notice the output as a failure
message?

When I look in SQL Workbench, I see two databases: cloud and cloud_usage.
I do not see a cloud_db database, which the document here implies should
exist (in a diagram at the bottom of the page, a cloud_db is listed):

Just messed around with running the commands again and now I do see two
