I had the same problems.
All seems working
but clamav-milter doesnt' produce output not to logs nor to e-mail headers.
I rebuilt the whole Sendmail package with milter enabled.
After that all works.

My Clamav-milter is working now with Sendmail for 2
days.
All goes Ok except for some strange behaviour
concerning to Exploit.IFrame.Gen.
One our user has subscription to mail list from SUBSCRIBE.RU site.
And the digest from there filtered and not delevered to him
with the words:
ter.sock, F=R, T=S:4m;R:4m;E:4m
and restarted Sendmail.
Nothing happened.
All goes as if without Clamav at all.
