Yes, I had the same issue and was not able to resolve it for squeeze in the
short time I had. I ended up upgrading to wheezy and everything worked as
it should. Mine is a test cluster so I didn't mind upgrading but I need to
resolve the issue for prod deployment.

The auth key needs to be copied to all machines in the cluster. It looks
like the key might not be on the 10.81.2.100 machine.
Check /etc/ceph for the key if you are running Debian or Ubuntu
I am just new to all this myself so I may be totally wrong but it seems
plausible in my head :)

Hi,
You mention early that one mon is spare but later say that you have 4 mons
running. Ceph explicitly requires an odd number of monitors so a quorum can
be established.
I think you should have 3 or 5 mons running not 3 and a spare.
