Does the deamon run under xinetd? Then you have to configure the only_from
in */etc/**xinetd.d**/**nrpe* to.

Yes, also it could be nagios use another configs location. Check: whereis
nagios.

Some more points:
Does the user nagios have rights to nrpe binary and config file?
Check nrpe.cfg for nrpe_user=nagios and nrpe_group=nagios.
To activate logging:
As default, nrpe log to syslog BUT you have to add daemon.debug to
/etc/rsyslog.conf :
**.info;mail.none;authpriv.none;cron.none;da
Have you checked the file rights under your document root ?
Your apache group need reading right.
