I think the remaining question is how common is this as a style? (I find
many examples in books but few online :) and does that warrant adding an
additional staff type?

and
pasted it into lilybin at: http://lilybin.com/v13cxt/1 and then tested with
2.18 and 2.19 the results mirror my local result with 2.18,2 on debian
