It could be bad barcodes on some tapes or faulty/dirty barcode reader. I
would think that if the barcode reader were at fault, it would affect
random volumes and not the same volumes each time. If it is always the
same volumes that have the issue, then maybe the barcodes on those tapes
are bad.

I suggest checking first if you are able to read the tape volumes labels
outside of Bacula.
on Linux shell type the following command :
mtx -f "TL Changer Linux Device" status
Please check the following example
mtx -f /dev/changer3 status
Storage Changer /dev/changer3:1 Drives, 46 S
If a labeled volume is missing from the catalog then either it was never added
or it was deleted.
Do you have any logs showing those volumes being labeled? That might give
some clue. Was it done using the "label" command or by automatic labeling?
