Obviously, if that doesn't help, I would uninstall pulse audio while
trying to sort out any alsa problems, but you have probably done that
already.

Are you quite sure that you have the mixer controls set correctly? I had
exactly the same problem with the ekiga softphone and it turned out to
be that I had not set the microphone capture mode in the alsa mixer.

In another thread I reported problems with aplaymidi with a 'Creative SB
AWE64 PnP' board. Once the right modules were loaded and a sound font
installed with asfxload, aplaymidi (and similar programs) just played a
single note. strace showed that the last call was
ioctl(3, SNDRV_SEQ_IOCTL_SET_CLIE
