Could you please isolate the specific request and response logs where you are
creating four extensions and actually having eight created? Are you seeing the
eight extensions with different Ids in the UI? You can use Reply privately to
author option to share the details.

Added note - even when running with other mutate requests, the behaviour is
not consistent. (e.g. Running the same code on a test account, sometimes
it'll create 8 extensions, whereas after clearing and running again, it'll
create 16).
