I have had the same error with our TSM environment. The PLM is AIX. And we have 5 * W2K library clients connected to a LTO. The problem was that when RSM was enabled they were all picking up the Library changer, which therefore stopped any tapes being mounted. The solution was to disable the RSM

A colleague of mine has done this about three years ago, moving from AIX 4.3.3 to Solaris 2.7... if I'm remembering correctly, it was via a DB backup-restore to flat file, with TSM 4.1.5.x.
