I found this also to be a problem simply when running jaunty as a qemu
or kvm guest.  Note, that the config for linux-image-virtual (linux-
image-2.6.28-11-virtual) does have CONFIG_HOTPLUG_PCI_ACPI=m, the module
is just not included in the -virtual package.

To reproduce:
- create a disk image to attach: qemu-img create -f qcow2 /tmp/my-disk.img 1G
- boot jaunty guest, with -server or -generic kernel so as to have access to 
acpiphp.ko module (bug 364916).  I've verified on 2.6.28.11.15
- inside guest: 'modprobe acpiphp.ko'  and 'modprobe virtio_blk'
- from kvm monitor: pci_add 0 storage file=/tmp/my-disk.img,if=virtio
  watch to see what slot this is put on
- inside guest: dmesg and cat /proc/partitions should show a new 'vd*' device
- from kvm monitor: pci_del 0 X 
  where X is the slot from the add
- inside guest: see message on console or dmesg:
  "modprobe virtio_blk ... Device 'virtio2' does not have a release() function, 
it is broken and must be fixed"

if acpiphp.ko were collected by the -virtual kernel image, wouldn't it
"just work" for you (except for loading the module)

Ie, if 
a. debian/sub-flavours/virtual.list had 'drivers/pci/hotplug/*', then 
acpiphp.ko would make its way into the -virtual image.  
b. That is the kernel image that is installed by vmbuilder, so your suggestion 
of loading via /etc/modules would work, and be easily enough enabled by a 
--execscript program in to vmbuilder.

I think that 'a' definitely needs to be done, a "virtual" kernel should support 
hotplug.
I dont know exactly what the best way to get the module loaded would be.

I agree with DaveAbraham's assesment above.  System works very well if
not docked on initial boot.  "Undock -> Power On / Boot -> Dock" results
in suspend/resume consistently working with or without devices attached
to the dock.  So at this point I have to make sure I'm disconnected from
the dock when I boot.

Regarding DOUBLE_CONSOLE_SWITCH, in my experience, setting it in acpi-support 
has no effect on this issue.  Note, also, you need to set it to 'true' for it 
to have any effect, '1' would not do anything.  You can see this at 
/etc/acpi/resume.d/65-console.sh line 65:

  This is the behavior that I've seen on my T60p also.  I dont have windows, 
and can verify that the cold boot to linux will fix the problem.   I'm not sure 
that a reboot won't fix it some small percentage of the time, but usually when 
I found it stuck like this I'd power of, wait a bit and power back on.
  So, other than sympathy, I can offer you a suggestion.  It *seems* fixed for 
me in 2.6.27-11-generic (from intrepid-proposed).  I also have a Thinkpad SL500 
that would exhibit this problem also, and since I've upgraded (maybe a week 
ago) I've not seen it again.

printing labels it gives the option to "print a sample".  I just saved
that example and attach here.  It crashes evince, but is fine with xpdf.

The reproduction instructions are complete, no local commits, the
revision that I picked to pass to bzr log is the one that 'bzr uncommit'
said could be used to re-apply the change with 'pull'. I just verified
with a new temporary user.

The attached script will create a bzr repo and reproduce the issue.

The -virtual kernel package should contain the acpiphp module.  The intent of
that package is to be used by virtual machines, and those machines should be
able to use kvm's pci hotplug.  That can be accomplished as I described in
comment 4.  Can we please have that done?

The documentation on setting up Eucalyptus right now
(https://help.ubuntu.com/community/Eucalyptus) suggests using
/boot/vmlinuz-2.6.. and /boot/initrd.img-... for guest kernel and initrd
respectively.  The problem is that on a ubuntu system that will not result in
functional pci hotplug support in the guest.

Etienne suggested changing CONFIG_HOTPLUG_PCI_ACPI to 'y' in kernel
config.

The other solution would be to get acpiphp.ko into the initrd that the user is
told to use.  That can be accomplished by adding it to
/etc/initramfs-tools/modules and running update-initramfs prior to 
'ec2-bundle-image' and 'ec2-upload-bundle'.

Sorry, should have stated that explicitly.  Presence in
/etc/initramfs-tools/modules gets the module modprobed before pivot_root.

 when  the system boots. The format - one per line - is identical
 to that of /etc/modules, which is described in modules(5).

To reproduce this behavior, run an instance that executes user-data as a
script (ie, the uec-images), like this:

If I ssh to the node controller, and look at
/var/lib/eucalyptus/instances/admin/i-4537075F/console.log I see that
the count went all the way to 4999.

Note, I've waited minutes after the VM wrote the data before getting
console output, it doesn't seem like just a (resonable) caching issue.

Also note, that the get-console-output takes an extremely long amount of
time (minutes).

I'm somewhat baffled on how this can happen.

That indicates that somehow, the 'metadata' member of self
(DataSourceEc2.py) does not have an 'instance-id' field.

That field gets populated in cloudinit/DataSourceEc2.py's get_data,
which does something like:

self.userdata_raw = boto_utils.get_instance_userdata(self.api_ver)
self.metadata = boto_utils.get_instance_metadata(self.api_ver)
return True

self.wait_for_metadata_service is an attempt to make sure the metadata
service is up before calling boto's get_instance_userdata and
get_instance_metadata methods to populate the members.

both of get_instance_userdata and get_instance_metadata also take
precautions to wait on the metadata service.  So, it appears that the
metadata service was correctly parsed / crawled by boto, and that it did
not have an 'instance-id' field .

I modified 'retry_url' in the cloudinit/boto_utils.py to have:

there and trying again will make this work.  I'll try that to verify.

Basically, the metadata service is up and responding but not populated
yet.

I've opened a Eucalyptus (and Ubuntu/Eucalyptus task).  This is really a
eucalyptus bug.  Ideally the instance would not be booted without the
metadata service up and ready.  That would be the ideal world.  As seen
in bug 308530, EC2 has an issue where if the instance looks too soon,
the metadata service might not be present at all (no route to host, or
failed tcp connection)

Eucalyptus seems to have a similar bug the metadata service responds
with http succes (code '200') before it is actually populated.

getting the first 64k.

I've run some tests with this patch applied on the system that Carlos
was running on.  The patch seems to do what was intended, to block and
wait for existance of metadata service with instance-id field or wait
for 20 seconds.

The result is that the instances still fail, but now, rather than
failing with 'consume userdata', they die like:

I've just tested, and failure message that the ssh client gives if the ssh 
server has no keys is just simply:
Connection closed by 204.236.215.149

Also, at that point ssh-keyscan will show:

I just ran test on uec that carlos was seeing this on, 200 instances
using mathiaz's lp:~mathiaz/+junk/uec-testing-scripts.

the end result shows 28 failures 25 of them had the following in their console 
log:
 | waiting for md at http://169.254.169.254/2009-04-04/meta-data/instance-id 
(try 19/20).

This shows up a great deal more on the data center uec than it does
elsewhere (Its occurs for me < 3% of the time, maybe even less than 1%).

Running tests for lucid rc, I came across one system that was not
accessible via ssh.

The console collected after waiting ended like:

I enabled 'message' level output from upstart in an instance and caught
this failure.  This has the cloud-init package attached to bug 566792
installed with upstart debug set to 'message' and mountall debug turned
on.  The console log here was attached 40 seconds after boot (normally
that would be easily enough time).

Note, as in the other attachment, it doesn't appear that the 'cloud-
config-cfg ssh' (invoked from cloud-config-ssh.conf) job is being run at
all.

Other information I did get was that ssh gave the following error message:
Read from socket failed: Connection reset by peer

and ssh-keyscan showed:
# 192.168.1.193 SSH-2.0-OpenSSH_5.3p1 Debian-3ubuntu3

I've tested Jamie's debs that he gave me access to. With the new debs in
place, an 'lsof -p $(pidof libvirtd) | wc -l' varies between 79 and 80
for me over the test of running several hundred instances.

I'm increasing the importance of this.  The reason is that after a
reboot, the 64k is almost certainly filled, even if nothing other than
kernel messages have been written there.  Thus, anything written after
that first reboot wont be seen by the user.

I've recreated this with a ttylinux based image, with modifications to access 
metadata service early in boot.
I'm attaching that instance here.

it will also boot in smaller than m1.small (ie, you can shrink m1.small to 1G 
fs and 96 M mem)

I do not believe that this bug is still relevant... in current versions
of cloud-init in lucid, you should not see the thrown error, but the
matching is not done case-insensitive.  I just committed a patch to
trunk (r185) that does case insensitive matching.

cloud init should be much more sane on failing over as "not a cloud
instance" than ec2 init was.  That said, it will hang a bit. cloud-init
should not be installed on non-cloud instance.

I just re-read the above and see that xen devices are currently ignored.
Given that, I do think it makes sense to also handle vmware (and
possibly even virtio devices the same way).

That said, we don't want xen, vmware, or any systems with multiple
network interfaces to give non-deterministic ordering.  If its not
already common, I would expect virtual machines with multiple interfaces
to be common in the future.  It would be nice to something to handle
that correctly.

Would it be possible to do add persisitent rules based not on MAC addr, but on 
bus location?  The idea would be that in a new VM, the devices would be 
attached the same, but different MACs.  if the rule said:
  "device on pci bus at :00:19.0 is eth0"
  "device on pci bus at :00:19.1 is eth1"
That might be persistent across VMs (or at very least could be made to be), and 
would be deterministic for multiple network devices.

I'm not familiar with python apt, so i'm aware this is heavy handed, but I did:
  strace -o out python -c "import apt; print

I see a long series of stat calls like:
stat64("/var/lib/apt/lists/us-east-1.ec2.archive.ubuntu.com_ubuntu_dists_lucid_Release.gpg",
 0xbff9faec) = -1 ENOENT (No such file or directory)

Those files dont exist.  However, what *does* exist is:
/var/lib/apt/lists/archive.ubuntu.com_ubuntu_dists_lucid_Release.gpg

The reason is that the install was done using 'archive.ubuntu.com', but
on first boot the mirror in /etc/apt/sources.list is modified to use us-
east-1.ec2.archive.ubuntu.com .  No subsequent 'apt-get update' has been
done.

Ok, so assuming "addressed based persistent naming" is desirable, I have
2 thoughts on how it could enabled:
a.) for all cards on the pci bus, use pci based persistent naming.
  This fails in the moving a card from slot 1 to slot 2 case as you
  mentioned above.
b.) for all devices deemed "virtual" use address based persistent naming.
  This fails if we can't determine something to be "virtual".  If we
selectively enable for known MAC addrs OR known drivers, then we'd miss
anywhere where the hypervisor implements a emulated device (pcnet32 or
e1000 in kvm) and selects a MAC address that we don't whitelist.

b, seems safer as we can have more strict control on the white list.

maybe it is sufficient to simply apply to these MAC addrs what we
already do to xen.  It just seems that then we are vulnerable to
device/driver ordering issues.

One other thing to consider is that kvm implements pci hotplug for
network devices (http://www.linux-kvm.org/page/Hotadd_pci_devices).  So
whitelisting of kvm addresses would result in inconsistent naming with 2
network devices being added and removed in somewhat arbitrary order.
