We have recently evaluated two scripting tools and would like to know
what those of you on the L-list think of each of them, if you have an
opinion.  The two vendors we researched are Boston Workstation and
Summit Healthcare.  Do you have either of these vendors?  Are you
pleased with the capabilities?  Do you have opinions as to which product
you think is easier to use, superior etc. and why?

Dawn,

 

Yes we script into the charge master usually once a year when prices
change or if we add new departments.  We use Summit Scripting tool.
When you write the script you set the field where the data is to go and
then you can watch it so you know what fields it is going into.  You
need to see a demo because it's just like someone entering the
information only the computer is doing it.  We do scripting for all
projects that require major data entry.

I posted some wristband NPR reports on Meditech's Report archives. Should be 
able to build from my report. 
You can print using the Zebra's ZPL II codes, and have Generic\Text printer 
drivers.
 
If you do not have an NPR report writer, I'm sure Mr. Cocuzzo (Iatrics) can 
provide some consulting work.

I can't get the Type Of Bill to print in box 4 of the UB04.  The TOB is
now a 4 character field and the four characters (eg 0123) are visible in
the spool file but when it prints on the claim form, it only shows three
(eg 012) with the right most digit truncated.  Does anyone have a
solution for this?  Thank you.

[MEDITECH-L] Magic NPR Select Field To Enter Free Text Message

I have an NPR report that I always print to MAIL when I run it.  I would
like to build a select computed field that allows me to enter a custom
message to the user that would be placed at the top of the report.this would
be a free text field similar to a CDS "E" type query.  Right now, I am
building the message directly on the report picture.

 

I am not sure if this is even possible to do, because I know that in select
fields used with the LI or CO operator, the cursor leaves that field after
the last response has been entered.  However, I know that the  key can
be pressed to exit an "E" type query.

 

I just wanted to post in case someone has come across this issue and found a
solution.  Any replies regarding this are greatly appreciated.

I need to calculate rehab times spent with a patient to be sure we are meeting 
our 180 minutes per day for our Inpatient Rehab Unit. I would like this to

Below is a message that I sent to Melinda Carson at FDB regarding the way 
the allergens pick list from FDB looks. I wanted to know if people agree 
with what I am asking them to do?

Melinda - Last night I was staffing and noticed that when I entered an 
allergy for Darvocet, I got the following choices:

Darvocet-N 50   Multiple
Darvon  Multiple
Darvon Compound 32  Multiple
Darvon Compound-65  Multiple
Darvon-NMultiple

Darvocet-N 100 wasn't even listed, so shouldn't it be? The other option is 
to just not put a strength and just have Darvocet-N. The same for Darvon 
Compound - why not get rid of the strength. I've never seen a strength 
listed for a med when it is listed as an allergy.


Also, Vicodin and Lortabs are listed kind of the same way
1  Vicodin Multiple
2  Vicodin ES  Multiple

1  Lortab  Multiple
2  Lortab 10/500   Multiple
3  Lortab 7.5  Multiple
4  Lortab Elixir   Multiple

Something like Zocor isn't listed separately for each strength, so neither 
should any drug - it makes no sense. Granted if we are looking at 
excipient allergies, it may be an issue, but like I said, I've never seen 
a strength written on an allergy in my entire life.

Dawn,

I have a script that updates the standard and fee schedule dates,
charges and prof components and can update the Alt codes.  We use Boston
Workstation for our scripting tool.

As far as QC I have all my scripts write detailed log files.  I also do
a random check of 10 - 15 procedure codes when the script is done.  Also
most of those fields are limited in what data they will accept and well
written scripts are less likely to make mistakes than humans.

I get the data from our Revenue Cycle analyst in an Excel spreadsheet. 
It is a lot faster and easier to key in the info to Excel.

[MEDITECH-L] Addendums

Hi Everyone,
We currently use Dictaphone as our OV for transcription.  Are
their any Magic sites that are able to get the addendums across from the
OV to Meditech? Our issue is the original report text is sent through
the OV interface again along with the addendum.  The report in Meditech
has the body of the report twice,who dictated and transcribed twice and
then the addendum.  Very sloppy looking.TIA!

[MEDITECH-L] Limiting a pointer query

Does anyone know if there is a way (via attribute, etc.)  to limit the
choices in a pointer query?  Example: The query is: Belongings were
checked by?..   The response pulls to the MIS.USER dictionary that
lists everyone.  I have been asked to limit that choice to RNs and LPNs
only.  Is this possible short of changing it to a group response and
listing all RNs and LPNs in the hospital?  Any ideas, please let me
know.

[MEDITECH-L] Patient Audits

Does anyone out there know of what I can use to audit users who has accessed 
patient records.PCI has an audit trail which works for patients viewed in 
PCI.  What I need now is something that will work for the NPR applications.  
Even if it has to be a seperate audit per application.  There are audit trails 
based on users but I need one based on patients.

[MEDITECH-L] Meditech LSS 837 ANSI4010A Medicare Claim

I have been testing with LSS over the past few days with the set up of the 837 
for Medicare for our clinics.   We have run into the issue of the Insurance 
Type codes.  I wanted to see how others are handling this.  I know there are 
two options .  The first being you can add all of the insurance mneumonics on 
page 2 and then enter the type code that goes with that insurance.  My question 
is who did that in your organization or did you assign the same Insurance Type 
code across the board with the exception of Medicaid?  LSS has also shown me 
another option where they have added a customer defined screen for users during 
the registration process to choose the appropriate insurance type codes.  That 
really isn't an option for us because we don't think they would "know" this 
information.  I would appreciate any assistance or any comments that pertains 
to the set up of this.  Thanks a bunch!

Rita, I have been told, by Meditech, that the UB04 will line up when
printed on the HP4250.  I haven't had a chance to test yet, but I will
be testing this next week.  I'll let you know what I find.

[MEDITECH-L] Nurses Coding Allergies - what about FOOD and OTHER

We are looking at EDM and implementing coding allergies via RXM for the whole 
house
Currently we have a multiple for drugs, food and "other" as well as a single 
Y/N for Latex

If we have nurses focusing on the routine in RXM we will need to create 
"allergies" for non drug items

What can I do for dietary.. the current free text food allergy field in the 
parameters will no longer exist!

Can any start me in the correct direction. I'm having trouble seeing the forest 
for the trees!!

[MEDITECH-L] Supplies to Charge Code match

Good Morning,

I would like to know how other sites handle supply charges codes.  Is there a 
one-to-one match between the Supply and the Charge/Procedure Code in BAR?  If 
so, are the numbers also the same in both Materials Management and the Charge 
Code Dictionary?

If you are doing grouping of many supplies to one charge code based on a 
specific category, how generic are your categories.  Are there many different 
screw categories based on the type, or do all the screws fall under the same 
charge code based on price?

Any help you can give me would be great.

[MEDITECH-L] PHA - BMV and equivalency dispensing/administering

Hi all, 
As we are expanding our BMV units to the ED and housewide, we are dealing with 
an issue that Meditech says hasn't really been an issue for others, and were 
wondering if you have encountered the following and how you deal with it. 
 
An med order will typically be for the largest size tablet which matches the 
dose size, let's say 80 mg of drug X BID.  We stock drug X in 80mg, 40mg, and 
20mg sizes.  The pyxis machine that the user is withdrawing the med from (or 
the main PHA sending the med), might not stock the 80mg tablet in that 
location, or might be out of the 80mg tablet, etc. and the nurse might need to 
pull 2 40mg tabs to make up the ordered dose.
 
Meditech says that the answer is to re-profile the order in whatever sizes are 
stocked by the particular area, but as you can imagine, that would be a 
never-ending task in a 450 bed hospital.
 
We are looking for some type of equivalency setting so that we can tell the 
system that 80mg of drug X = 2*40mg tabs = 4*20mg tabs, and if the barcodes on 
any of these are scanned, it should understand that it is still med X, but that 
a different # of tablets would need to be scanned.
 
Any thoughts?

[MEDITECH-L] Pharmacy Labels

We are using 4x1 labels in the Pharmacy and I have been asked to see if
I can increase the size of the printed output.  I hope that there is
someone out there who has had success in doing something similar.  We
are using Datamax I printers with the Seagull driver.  I have tried
changing several things, including adding a Print macro but everything
I've tried doesn't work.  Basically when I mess around with the
Chars/Inch, making it smaller than 17 (which is the current setting) not
everything is printing on the label. Meditech hasn't given to work with
and I've tried all of their suggestions but nothing has worked.  Has
anyone out there been successful in getting a larger print size for
pharmacy labels?  If so, I would really appreciate you sharing how you
accomplished it.  Thanks in advance.

I have used both the script tag and windows GUI version of  the summit
scripting toolkit to script into Meditech magic and other windows
applications and the program works very well for relatively simple,
straight forward scripting projects. The script-tag Meditech interface
works particularly well. I use this daily to post cash and mass tag
accounts. 

Some of the more complicated windows functions are a bit cumbersome and
not well documented within the application toolkit or on the web site,
but thier tech support is adequate and can assist with these issues or
will typically escalate to a developer.

Where the application falls short is the dictionary sync tool, which we
were very interested in when this was pitched to us. However, after
working with the application, it falls short of expectations. If you
need more detail about either the scripting toolkit or the in-sync tool
please feel free to contact me.

The Medication Discharge Report that is under Reports in 5.5 look more like a 
MAR, meant for a patient to be transistioned into a SNF. I am not sure if it
is different in 5.6, but when I asked Meditech to provide me with a zcus. of 
the report so that I could try to edit it, the .ZCUS report was
created out of several fragments or macros that were not separately provided 
and such that I could not edit the report for our use.
If 5.6 has an updated version and it works and can be edited in the report 
writer, please let us know. Thanks Cindy

This is the biggest challenge we've encountered in Meditech, and I think
other hospitals will agree.  We started out in 1998 torturing our nurses to
attempt to complete this task in the units.  We did it for almost 2 years
but finally felt that at the time, the technology wasn't out there to help
this process so we kept a revised paper flowsheet where vitals, hourly I/O's
and drips were kept, among a couple of other items I think.  Staff were
responsible for still inputting q4 VS and end of shift I/O into the computer
to keep the integrity of the on-line record for when the patient transferred
to/from other depts where everything was on-line.  Now with the Iatrics
Visual Flowsheet and even Meditechs improved, albeit primitive compared to
Iatrics, flowsheet, the ICU challenges are much more easily addressed.
However without bedside computers in every room, we have decided to keep
with a modified paper flowsheet until we get computers in each ICU bed.
Another strong recommendation would be a monitor interface for dumping the
data into Meditech, but I feel you can do it without the interface as long
as other conditions are met.   Good luck!

We are planning a May implementation of NUR documentation.  Our ICU units
feel there is no way they can document their Q5-15 minute Vital Signs or
their drip titrations online.  They want to keep these items on paper.  With
the titrations they don't feel they can see trending with the system.  We
are Magic 5.5 SR2 and plan to use the process flowsheet routine in Meditech.
Anyone have any suggestions or things you do to assist in this process or
just leave on paper?

"Don't give up" is all I have to say - you'll need very strong
administrative backing to support your decision to move forward toward a
wireless EMR.  I've been through the hell you speak of and survived.  Even
though we're not completely paperless for our unit, it's because we don't
have PC's at the bedside.  But we're fairly close to paperless.  
Your docs will either come around to embrace it or give in and be
quiet.  I've implemented NUR 2 different times at 2 different facilities but
utilizing essentially the same medical staff where some are very set in
their ways.  The docs won't all be converts but if admins sticks to their
guns, you'll eventually find your success.  And admin is KEY to your success
in the units.  If you have specific questions, don't hesitate to contact me.

We have are getting the same feedback from our physicians.  We are C/S 5.4,
SR2 and have been live with PCS since May of 2005.  They have voiced the
same concerns and I have tried EMR reports and other options, but like you,
they do not like it.  They believe they are missing key information because
there is so much information to look at.  The "My Panel" option has helped a
little, but any other ideas from the L would be greatly appreciated!

Help!  Some of our Critical Care physicians state they can not get the
"whole picture/story" of their patient's progress/problems to successfully
assess and order.
I have been directed to poll the "L" and gather percentages for each of the
questions below.
1. Do you paper document only.
2. Did you document in Meditech but had to return to paper due to this
problem.
3. If you document in the NUR module, how are you satisfying your physicians
need to see the "whole picture".  We have used PCI to group several queries,
and have Iatrics VFS to offer but can't seem to tailor this to their needs
either.

We are Magic 4.9, this summer will be 5.5 sr?.  We are a small hospital
without residents and interns.  We do not want to return to paper!

with Meditech for 3 years.  The report works for now but I know that I
have to find a better solution for down the road but I really needed
something to get out to the agency.  I would like to keep the same
format but if you have any suggestions for making it faster without

[MEDITECH-L] Security and patient access

Hey all,

 

I am new to the Meditech CS world and am currently working on security for
the clinical personnel in my facility.  In our current system, I have the
ability to limit patient access based off of not only the provider group one
belongs to but also the location of the pc that one signs in to.  For
example, a RN on our Women's Health Care would have access to patients in
our OB, L& D and of course the nursery departments when signed on to a
computer in any of those locations.  However, if she floated up to our
Critical Care Unit she would then have access to patients in our CCU and
also our PCU.  

 

I am looking for a way to do this in Meditech and have not had any luck.  I
know I can set a user or profile to be restricted only to location but that
is only one location.  Does anyone have any ideas?  Thanks so much for your
help.
