Review of updated Triage documentation

Hello all,

Our triage documentation is several years out of date.  We've put together
a new doc with some changes to the process. This should not be too much of
an impact on your workflows because it is mostly an update to the reality
of what shorter release cadence has already forced upon triage owners.
