psmisc/procps maintainer didn't want it there, moreutils probably isn't
the right place and a python dep probably wouldn't be wanted.

I assume that the plugin simply isn't ready for usage with OFX yet. But
that's a question the upstream authors Thomas and Martin should be able
to answer for sure (CCed).

It is because you are not using OFX DirectConnect...
Just activate the account in the account view, then goto menu 
entry "Account/Online update using AqBanking".

No (not yet - a change in that area is still sitting in my sandbox)

If you use KMyMoney CVS HEAD, you can use Tools/Plugin to see if the plugin 
is even loaded ... but you're on debian, so I assume, you're still on 0.8
