It is strange... mcqd was merged in the trac ticket #16083 but
apparently the list of optional packages was not updated. The same
thing happens for pip (trac ticket #16479).

Right now, you can do:
 1) download mcqd-1.0.tar.bz2 which is linked from
http://trac.sagemath.org/ticket/16083
 2) put it in SAGE_ROOT/upstream/
 3) run "sage -i mcqd"
