On MacOS Mojave you have to manually install the system headers, either go 
to  /Library/Developer/CommandLineTools/Packages and click on 
macOS_SDK_headers_for_macOS_10.14.pkg or run 

open  
/Library/Developer/CommandLineTools/Packages/macOS_SDK_headers_for_macOS_10.14.pkg

in a terminal.
