John, on the strength of your recommendation I bought vol 2 of Cohen's book. I think the worked example is actually in volume 1, at least according to the contents list in Vol 2. It's at the end of 'PART 2' which is in Vol 1. I'm waiting for the postman to get back after Christmas and deliver Vol 1.....
