in the tradition of BSD Mail, which is a monolithic program.
Thus i'm also not sure that S-nail is a way forward :)

Short answer: no.
In the far future (starting with S-nail v15.0, not before (year)
2015) you will at least be able to address MIME parts
individually.
It would be nice to end up being able to extract a single
attachment / whatever part from a specific message inside
a folder, but i'm not sure and it is anyway a pie in the sky.
Until then your only option is to define mimetype handlers for
viewing and whatever else you can do with sending ('set
pipe-content/subcontent=action', e.g., 'set
pipe-application/pgp-signature=@' for treating signatures as plain
text), but one should be careful to do so at the moment, because
all such handlers execute once the message is viewed...
And personally i don't really like the concept of nmh(1) in
a normal UNIX environment. Things are different on Plan9 (9front,
9atom) with its mail filesystem, which lets you address the
individual parts of a message as normal files. That is, i never
have used nmh, but i don't think there is an active (per-user)
server which acts as a caching instance or something? So i would
go grazy thinking about the work that happens in the background..
P.S.: note that "offline IMAP" may even change in the future and
may end up as a special mode of database operation. Today sqlite
is everywhere, and it wouldn't it be nice to organize messages in
a nicely layed out database, automatically, splitted into headers
and (compressed?) message (blobs)? Like that sqlite command line
tools can be used to act on the database, and then we could offer
an interaction with a specific unique DB message-ID. But this is
even more hypothetic. (Yet i like the idea.)
