Hi all,

Tom, Jasdip and I have put together a new version of the draft based
on feedback from the list and IETF 117.

In summary, the changes are:
* status changes from an informational to an update
* reserves two consecutive underscores for IETF definition at later
time (such as in a versioning draft)
* provides instruction on collisions of identifiers and recommends
against further use of underscores in extension identifiers
* codifies the use of query parameters with extension identifiers
* codifies the use of bare extension identifiers
* recommends the use of extension identifiers for IETF owned extensions
* clarifies that the document is not defining a versioning scheme
* discusses backward compatible and incompatible extensions
* notes current extensions that do not follow the extension guidance
