You can create an xinetd service in the firewall which runs telnet, and
have your internal hosts telnet to this port, then issue an open
command.

Yes you can do NAT. IPSEC gateways have trouble with NAT when the IPSEC
gateway is behind the NAT router. If the same box is doing both IPSEC
and NAT, you should have no problems (given an adequate NAT ruleset).
