Won't the password and restricted options accomplish this within LILO?

If I understand correctly, you want to protect single user boots from
strangers walking up the the computer.  (Correct?)

Look at moving from lilo to grub.  Grub has a password facility.  I
think you can set it to let a normal boot go by but require a password
to change to, say, a single user boot.

If physical security is lacking, you might also want to look at having
a BIOS password and case intrusion switch, not letting a CD-ROM or
floppy boot, etc.

Tricky stuff.
