OK, I've come across this before.

You need to move the cables around. I'm e-mailing a customer who had this
problem as of this moment who know exactly the combination required. I think
it was 1 and 3 you need to use, not one and two, but I'll know for sure
within a day. Sorry to take that long, but he had exactly the same thing.
