Have you checked the perms on the squid cache?  Squid normally runs as
user/group squid, so chown and chgrp the intended cache directory,
then do squid -z again ... see if that helps.

What version of squid?  2.3 requires a cache_dir "type" to be set,
which the docs say should normally be UFS.  Look for cache_dir in
squid.conf for the syntax.
