Hi you don't add 

>AddIcon /icons/php3.gif .php3
>AddIcon /icons/php3s.gif .phps

to srm.conf you have put the above in httpd.conf

then restart Apache by /somewhere/apache-1.3.6/bin/apachectl restart

and it should work
