I have the same setup, and never encountered a problem: DOS on C:
(primary), NT on D: (logical 1st hd), Linux RH on hdb1 (primary). LILO
boots either Linux on hdb1 or the NT loader (with more options) on C:.
Of course, I made sure to save a copy of the MBR before installing LILO
there. IOW, I didn't install LILO on the MBR at the end of RH
installation. 

I re-installed NT twice without changing the LILO setup: no problem
there. I have compiled several kernels and changed LILO options a number
of times since: again, no problem there. The advantage with our setup is
that you don't need to recreate the boot file for NT boot.ini everytime
you run LILO. Everything can be done from within Linux.

The only problems I had with NT were: 
1) the existence of primary DOS/NTFS partitions on other disks (the ext2
partitions are no issue, since NT doesn't see them). Since NT still uses
the dumb letter scheme to refer to partitions, this can cause an awful
lot of stupid problems. My solution (I have 3 hd's) was to make all
other partitions except for C: logical.
2) Having installed LILO after the first NT setup, NT compalined at next
boot that it didn't have a "signature" on C:. What this means is beyond
me, but after having put its signature wherever it wanted, NT never said
another thing.
