Yes, I have used a SCSI terminator. Is my problem related to the connection cable and/or terminator? I am not quite familiar with these stuff but the cable and terminator that I used fit well with the host adaptor and the tape drive.

Lam, you're getting lock-ups. It's been my experience that lock ups = hardware problem. With SCSI devices I find it is usually incorrect set-up, but yes, it can also be a faulty cable, connector. It can even be a resource conflict with the card and another card in the system.
It's hard to track down I'm sure. I would start by making sure there are no resource conflicts.
