How do you implement user account lockout in Redhat Linux?

i.e. a user has "x" number of failed logins in a row and then the 
account is locked out for some specified period of time or until an 
administrator unlocks it.

(I know some will say "That's a bad idea because of DoS issues!". 
However, it is a *requirement* for many situations, and I just need to 
know how to implement it.)
