The only thing I could think of was that I noticed the surface of the DT 
shifter lever the previous owner had mounted to the bar end pod was not 
mating totally flat to the bar end pod, enabling some wiggle and 
instability in the lever.
Also, the bar end pod showed alot of wear to that raised square bump-out on 
the inside surface, so it won't take an adapter because the edges are worn.
 
So I am getting a new shimano pod + adapter installed (adapter similar 
to the one's Riv sells). The LBS had some. They fit like a glove. Then a 
proper mount of the lever. And I hope that does the trick. Otherwise I 
don't know what to do next. Start tearing my hair out. Crying may help.
 
 Maybe new chain and cassette. I hope this doesn't get expensive.
