Rob, Derby's .... yeah ... I've always wanted some Birk shoes ! They even make 48N.
I've got 3 pairs of the sandals ... one upper dating back to the early 90's..... nothing like resolable footwear :) I'm riding in the narrow ones ... which I didn't wear much as for walking they were not as comfortable as the regular ones.... but for riding ... the narrows are perfect.
