"Rivendell" headbadge and downtube decal would suggest a custom. The
"hourglass" mid-fork braze-ons would suggest within the last 4 years, I
think.  That does seem to be a sage green which is a custom color, rather
than the Atlantis seafoam green.

The other interesting bits are the "simple" lugs which do not have the shape
of the current lugs (though the "back end" on the top and down tubes seem
more complex). And I think those are the longer tips on the fork crown,
again suggesting earlier.  It might be an early custom with a repaint and
some newer bits brazed on.
