Adding to Ted's barend comment - 

AND the RD shifter easily converts to friction mode with the FD barend in 
friction only (at least the 1980's and 90's models I've used).  And then, 
of course, there's the RBW Silver shift levers with barend mounts. 

Hmm...a full RBW drop bar shifting system will soon be possible
