I used Kelly’s for several years on a commuter with half step gearing where
they were particularly good for double shifts. I set mine up so that I
could shift from almost all hand positions on a drop bar: flats, ramps,
hoods, and hooks. With my cruising cog in the center of the cassette most
of my shifting was simply flipping the left lever back and forth. I used a
7 speed cassette and half-stepped the middle 5; the extremes were for
downhills and steep uphills respectively. 25” wheel.

But I never liked them for offroad; I found bar end shifters to give a more
secure control of the shifters over rough terrain. I take it others use
them successfully on rough surfaces? I’m tempted to try them again.
