I've installed and adjusted many, many cup and cone bbs, from horribly cheap ones that were very hard to adjust for proper tension to top of the line like Suntour GG and Dura Ace that were easily installed with perfect adjustment. It does help to have good tools -- and I've still got an array of pin wrenches and lockring wrenches and pliers and fixed cup wrenches. But I recall removing, installing, and adjusting the bbs on my Raleigh Sports and Raleigh Sprite, and on Indian roadsters, using a pipe wrench for the fixed cup, a Vise Grip for the lockring, and a nail and hammer for the adjustable cup. Got 'em pretty close, anyway.
But good quality cup 'n' cones aren't hard to work with if you have decent tools. Really though? The easiest and IMO most efficiently designed (performance, longevity, cost -- so I imagine -- and ease of maintenance) off bbs is the Ashtabula; always a doddle to install and adjust, even on cheap bikes. The cheap cottered bbs and cranks were a horribly less efficient design, IMO.
