Looks like you've asked to have braze on mounts added to the seat stay or 
were they always there? The QB/S1 drop out has a single rack/fender mount. 
I got around it by drilling and treading a hole in the rack strut above the 
original mount.

To Max S,

The Phill Wood hub has "axle" end caps of different lengths which can be 
taken off with an Allen wrench. They end caps are not cheap, but nothing PW 
is. There are a pair for 120mm and another for 130/135mm. They work great, 
easy to change.
