I beg to disagree.  The Audi "unintended acceleration" issue was proven
-- PROVEN -- to be user error and no fault of the car at all.  TCO can
cause problems that have nothing whatever to do with user error.  

It is not user error if a bicycle with high wheel flop becomes unstable
at very low speed, for example on a 15% grade, requiring a lot of wheel
turning to maintain balance.  With a lot of TCO and a foot that's long
enough -- and although Fats Waller might not agree, the fact that "Your
Feet's Too Big" ( http://www.youtube.com/watch?v=lE-maxEOc4Q ) might be
your affliction, but technically not really your fault.
