I'd suggest a visit to the LBS.  They can tell if it's a cup'n'cone (old 
traditional style) or pressed in bearings.  Have you ever replaced the 
grease in the bearings?  If not, after 4 years of flogging about the 
mountains they are due for some attention.  Cup'n'cone are basic, low tech 
and as long as the contact surfaces aren't damaged can be re-packed for 
years.  The original XT hub on my Atlantis is way over 60,000 miles.  I 
clean & re-pack the bearings every couple of years & at this point am just 
curious how long it can last.  It's certainly earned it's keep.  

Replacing sealed bearings does require some expertise beyond the average 
home mechanic.  Some shops may even shy away & sell you a new wheel.  Given 
the kind of beating yours have taken, it could just be time for a 
replacement.  The original rear rim on my Atlantis was looking pretty wonky 
after 5 years and it didn't see the off-road workouts yours does.  I had a 
new Mavic A719 laced onto the old hub & it's still going strong.  Someday 
the brake surface will wear out but until then I'll leave it go.
