Richard Sachs posted a short little description today of how the original 
Rivendell lugs came into being.  Turns out Grant commissioned them while he 
was still at Bridgestone, but Bridgestone USA closed before they were used:

Also a cool photo of the prototype HT/DT lug. Supposedly the protos were 
hand carved from french blanks (thus would need metric tubing) and have 
extremely wonky angles, so according to RS they can't really be used to 
make a bike.  All the same, I wonder what such a bike would look like!
