I would say either Tube defect from the tube manufacturer Overheating during the brazing process with a lug having a ID too large and out of specification
The small crack coming out of the large upper crack is puzzling The lug seems to have excessive space to the tube Second the suggestion to contact RBW, but as 2nd owner????
