There may be nothing wrong with the bike. I noticed with my kids when they 
learned to ride a bike, they had a certain pattern of pedaling that they 
preferred: to stand up and pedal for a number of strokes, then to sit and 
coast briefly, then to stand and pedal again. It may have something to do 
with the bike being a significant fraction of the weight of a small child, 
and the power produced by their little legs being very limited. It's like 
one of us having to pedal a hundred-pound bike. I think this may be just a 
natural way for small children to ride a bike. 

And I recall my son never changed gears either, and he never wanted his 
saddle raised to the "proper" height we would use as adults. 

If he's having fun, maybe just let him do what feels natural? :-)
