There may be nothing wrong with the bike. I noticed with my kids when they learned to ride a bike, they had a certain pattern of pedaling that they preferred: to stand up and pedal for a number of strokes, then to sit and coast briefly, then to stand and pedal again. It may have something to do with the bike being a significant fraction of the weight of a small child, and the power produced by their little legs being very limited. It's like one of us having to pedal a hundred-pound bike. I think this may be just a natural way for small children to ride a bike.
And I recall my son never changed gears either, and he never wanted his saddle raised to the "proper" height we would use as adults.
