Last time I was at Rivendell, I bought the Yokozunas they sell in both smooth and threaded (I needed threaded). Their spacers and cupped washers were really easy to set up and adjust. When I was there, I noticed all the brake pads were toed in to the front to keep them from squealing which is the opposite from what I had always done in the past. Keven suggested that I take a piece of sandpaper, fold it in half, place it in the back and compress the brakes to get the angle. I combined that tip with Grant's bike geek tip on the Blug about using a section of inner tube as a parking brake. It was just the right pressure so I could align everything with the brakes engaged and then crank them tight in position. I can't speak to longevity of the pads but I have had no problems with squealing brakes since.
